Online Application Instructions

Step 1: Log in to the online application system via Microsoft Forms.

If you have already applied to AACC: You can log in to Microsoft Forms with your MyAACC username and password. If you have forgotten your login information, call the technical service desk at 410-777-4357 (HELP) for assistance.

If you have not applied to AACC: You must complete our general credit application before you can begin your Surgical Technology application.

You must use your MyAACC student login account information to access the Surgical Technology Application Microsoft Form.

The Microsoft Forms application can be found on the Health Science Applicants page.

Step 2: Begin a Surgical Technology application.

To apply to the program, you must submit a completed Surgical Technology application by the application deadline, Aug. 30.

The application includes demographic information, academic/admission requirements and the certification and signature page.

Note: You are not able to save your progress on this online application. Only begin the application when you are ready to answer all required questions in one sitting.

You will also be reminded to submit your official high school transcript, GED® transcript and/or college transcripts to the records office, depending on how you answer the application questions.

Transcript evaluations for prerequisite courses not completed at AACC are conducted by the records office. Your academic record and the Surgical Technology Microsoft Form portal are not the same online system. If you confirm that your transcript evaluation is complete and your transcripts are on file, you do not need to contact our office to update your Surgical Technology application.

Step 3: Check your student MyAACC email account.

Once your application has been submitted, check your AACC email for confirmation. It can take up to one business day for confirmation to be sent. During the review process, additional notifications about your application will be sent to your AACC email account.
